If you and your spouse or domestic partner are enrolled in a Mosaic medical plan, you have an opportunity to earn up to $800 in Rewards Cash (formerly Pulse Cash) each by completing any combination of the activities listed below.
Use Rewards Cash through Personify Health (formerly Virgin Pulse) to:
- Purchase fitness items (e.g., apparel, accessories, electronics, personal care items)
- Redeem gift cards (e.g., Prepaid Mastercard, Amazon, Home Depot)
- Donate to non-profits (e.g., Special Olympics, Clean Water Fund, Habitat for Humanity)

Online Health Assessment
This short, confidential survey assesses your health across seven factors. Get a personalized report and actions to take. Complete the assessment each year to earn your incentive. Incentive can be earned one time per calendar year.
Personal Health Screening
Get a snapshot of your cholesterol, blood pressure, blood sugar level and BMI to help you track progress toward your goals. Incentive can be earned one time per calendar year.
Complete Your Personal Health Screening with Labcorp
The preferred lab for walkin appointments or homekit orders is now LabCorp, changing from Quest Diagnostics. LabCorp offers flexible options and easy access to screening services. For more information please read the Personal Health Screening Instructions.
Track Steps or Active Minutes
Download the Personify Health app and follow the prompts to connect your activity tracker. Once connected, you have the option to track 7,000 steps a day, 15 active minutes, or 15 workout minutes for 20 days a month to earn $10 in Rewards Cash each month. The incentive can be earned 12 times per calendar year.

Preventive Care Visits
Preventive care is one of the easiest ways you can help yourself stay healthy. Complete any combination of the following exams in 2025 and earn $100. Incentive can be earned two times per calendar year:
- Annual Physical
- Prostate Screening
- Colonoscopy
- Preventive OB/GYN Exam
- Mammogram
- Dental Cleaning
- Vision Exam

Cigna Coaching Program
Work with Cigna's Personal Health team to make progress toward your personal health goals (i.e., nutritional counseling, tobacco cessation, weight management). Call Cigna at 800-244-6224 or visit www.myCigna.com to get started. Incentive can be earned one time per calendar year.
Hinge Health Exercise Therapy Session
Complete a personalized exercise therapy session designed to help you conquer pain, recover from injuries or surgery, or stay healthy and pain free. Sessions can be completed in 15 minutes or less. Learn more about the benefits of Hinge Health and how to get started here. Incentive can be earned only once during your employment at Mosaic for a total of $100 rewards cash.
Community Wellness Events
Participate in organized community wellness events (i.e., Relay for Life, American Heart Association Walk, 5K). Once you’re done, sign in to your Personify Health (formerly Virgin Pulse) account and navigate to the Rewards page. Then click on Participate in a Community Wellness Event to complete the attestation form. Complete the form to earn your $50 Rewards Cash! Incentive can be earned four times per calendar year for a total of $200 in Rewards Cash.
Company Step Tracking Challenge
Looking for a little friendly competition? Watch for details on step challenges later this year. Participate in a fun step tracking challenge where you can earn prizes! In order to earn this incentive, you must track your steps at least once a week for all weeks the challenge is active. Incentive can be earned up to two times per calendar year for a total of $200 in Rewards Cash.
Livongo/Teladoc Health Condition Management Program
You can earn up to $300 in Rewards Cash, $100 per program, if you qualify and activate your diabetes, hypertension, prediabetes or weight management programs. To activate a Livongo/Teladoc Health program, you must enroll and utilize the device provided with the program(s) you qualify for. Incentive can be earned only once per program.
