GEM SLEEP (Virtual Sleep Clinic)
GEM SLEEP is a virtual sleep clinic designed to help members at risk for sleep apnea get tested and treated quickly from home. After taking a sleep test in just one night using the WatchPAT® device, GEM clinicians will review your results to provide a diagnosis along with personalized treatment recommendations.
If treatment is needed, it may include a CPAP machine or oral appliances from GEM SLEEP. The GEM team will guide you through every step, from getting started to ongoing follow-up and progress tracking.

- Add the home sleep test to your cart and schedule a brief telehealth visit.
- Enter your health plan information to confirm eligibility.
Note: Some out-of-pocket costs may apply depending on your plan, but you can use your Health Savings Account (HSA) or Flexible Spending Account (FSA) to pay.

Livongo by Teladoc Health (Diabetes, Weight, & High Blood Pressure Management)
The Teladoc programs are available at no cost to Mosaic enrolled medical plan members. Your personal information is always kept 100% private, and you have access to a support team via phone or email for any questions you may have.
When you enroll in any of these programs, you become eligible for the following benefits:
- Diabetes Management: For members diagnosed with diabetes, you will receive a smart touchscreen glucose meter that automatically uploads readings and provides personalized tips, along with unlimited test strips and support from dedicated coaches.
- Weight Management: For members needing support with weight management or pre-diabetes, you will receive an advanced smart scale, one-on-one coaching, tailored meal & nutrition plans, guidance on healthy habits, and more! This comprehensive support is here to help you manage your weight and prevent diabetes.

Hinge Health (Virtual Physical Therapy)
Hinge Health provides virtual, physical therapy designed to help you conquer pain, recover from injuries or surgery, or stay healthy and pain free. The program is free for you and dependents age 18 or older if enrolled in a Mosaic medical plan.
The benefits of Hinge Health include:
- Access to a physical therapist who will guide you through your recovery, along with a health coach for chronic pain management if needed.
- A personalized exercise therapy program tailored for you. Each exercise session can be completed in 15 minutes or less, making it easy to fit into your daily routine.
- A women's pelvic health exercise program designed to address pain, discomfort, bladder control, and other pelvic health concerns.

Milk Stork
Milk Stork is the first-ever breast milk shipping company for breastfeeding moms and easing the load for hardworking parents. Pump, Pack, Ship or Tote and get your breast milk where it needs to go. The Mosaic Company is offering Milk Stork as a benefit to our breastfeeding employees and their spouses/domestic partners who are on business travel. Milk Stork can also be used for Surrogacy Support and Relocation of your frozen milk supply when moving.

Tobacco Cessation Program
Through Cigna, you have access to a tobacco cessation program. You'll be able to schedule coaching sessions, speak to professionals over the phone, or participate in an online program.
Employees and their spouse/domestic partner are eligible to receive a tobacco cessation premium credit for their medical insurance upon completion of the program.
